Sensory Profile

Sensory Profile Assessments, an integral tool at Lifeworks Occupational Therapy, are designed to evaluate an individual’s sensory processing patterns. These assessments help us understand how sensory information is perceived and processed, which in turn influences behavior and learning.

NDIS Access Application Process

NDIS Access Application Process

These reports will need to outline the following:

Your medical conditions (physical, neurological and/or psychiatric diagnoses).

  • History of treatments and/or therapies received.
  • Your condition(s) are permanent and no further treatments that can cure it (i.e., exhausted all medical treatment options; you now have a permanent psychosocial and/or physical disability).
  • Once you have obtained the medical evidence report(s) and your NDIS application form has been completed by your nominated doctor, please go to Step 2

Disability Support Pension Application Process

Obtain medical evidence reports from your doctors (e.g., GP, specialists etc).

Obtain medical evidence letters from your doctors (e.g., GP, specialists etc). These reports will need to outline the following:

  • Your diagnosed medical conditions (i.e., must be formally diagnosed by a medical practitioner).
  • Your medical conditions must be:
    Fully treated (i.e., everything that can reasonably be done to improve your conditions has been done).
    Fully stabilised (i.e., your medical conditions will not likely improve over the next 2 years).
  • Your medical conditions must cause you to have a functional impairment. Having a functional impairment means there are things you cannot do in your daily life, including work. An occupational therapist can help you determine your functional impairment(s).
